Gyserfilmen vil udspille sig mellem den første 'Saw' og 'Saw II', hvor John Kramer aka Jigsaw stadig er i live. I et desperat forsøg på at kurere sin uhelbredelige kræft rejser Kramer til Mexico, hvor et firma har lovet en mirakelløsning. Men da Kramer finder ud af, at det hele er en fidus for at lokke penge ud af uskyldige, dødeligt syge mennesker, planlægger han et grusomt spil for at få en blodig hævn.

Da orkernes verden, Dranor, ødelægges af onde kræfter, åbner de en portal til menneskenes magiske rige, Azeroth, hvormed horder af orker invaderer det ellers så fredfulde land.
