Three Italian retirees embark on a journey to find a new country to live in.

De unge guerillakrigere er udstationeret langt ude i den latinamerikanske ødemark og opererer efter ordre fra en fjern instans, der blot kaldes for Organisationen. Der er ingen regler, kun ritualer. Der er ingen ledere i deres formløse hær, og heller ingen egentlige hovedpersoner i filmen - udover det amerikanske, kvindelige gidsel og den malkeko, som gruppen har ansvaret for at passe på. For Sama er lavet af en kvinde, som selv boede midt i krigens elendighed, uden nogen steder at tage hen. I fem år lod hun kameraet køre, dag og nat, gennem liv og død. Kvinden hedder Waad Al-Khateab, og stedet er Aleppo i Syrien, hvor hun lever sit liv, bliver forelsket, gifter sig og føder en datter. Og imens bliver byen ødelagt bid for bid, og hun og hendes mand (en læge med et overjordisk mod) forsøger at redde så mange som muligt.
